The Outreach Worker is primarily responsible for providing assistance to displaced and unhoused community members in Kent and Sussex County. The Outreach Worker is part of a team who engages in on-site outreach programming at the Milford and Hopes and Dreams Site, street-based outreach programming, encampment outreach programming, conducts screening, provides internal and external referrals, and provides ongoing case management. BCCS Outreach programs offer an array of support and resources, including anonymous needle exchange services, free HIV and HCV testing and counseling, pregnancy testing, overdose prevention counseling, information related to reducing risk, referrals to drug treatment, health care, and social services, and access to basic needs such as clothing, showers, and laundry services. The Outreach Worker acts as a liaison between the clients and services, assisting the clients with treatment entry and navigating the steps necessary to successfully link clients to helping professionals. The Outreach Worker is responsible for providing quality, sufficient and appropriate outreach services and for maintaining high professional standards.

Be responsible for locating, identifying, and building relationships with people living unsheltered for the purpose of providing immediate support, intervention, and connections with homeless assistance programs and/or mainstream social services and housing programs.

Develop engagement and case management activities to provide essential services to connect participants with emergency shelter, housing, and critical services.

This is a primarily community-based position, so work will be performed outside of an office setting on a regular and consistent basis

Qualifications

REQUIRED: High school diploma or GED

REQUIRED: Valid driver's license with 2 points or less

REQUIRED: 1 year experience in Outreach or Social Services

REQUIRED: Must be a peer.

REQUIRED: Must be in recovery for a minimum of 2 years
